100% agree with your opinion. There should be safety all the time and implementation of safety rules and the facilities are the responsibilities of the tour operators and the boat house owners.

How many accidents and lost lives does it take for the authorities and the stakeholders in the tourism industry to wake up and start implementing safety standards?

I am of the strong opinion that the tourists are not to be blamed for their actions; it is the responsibility of the tour operators / houseboat operators to enforce and implement safety precautions... especially in this case at the time of boarding and getting out of the houseboat. IN SHORT, ALL TOURISM VENDORS MUST BE WHOLEHEARTEDLY COMMITTED TO THE SAFETY AND WELL-BEING OF THEIR CLIENTS.
