Abstract

Migraine is referred to a throbbing pain with pulsation in the head. Usually pain is felt on the half side of the head. Among various types of headaches. These are basically divided into two categories one is primary headaches and second one is secondary headaches. Primary headache is due to local reasons, like nerves or any regional factors MIGRAINE is primary headache. And Secondary headaches are due to some other related medical conditions. Migraine is usually diagnosed clinically; no such blood examination or scans can help in early diagnosis. It is diagnosed clinically by symptoms and it has one salient feature called AURA.

Introduction

MIGRAINE is characterised by presence of throbbing pain and AURA before pain attack . and majorly seen in the temporal region of the head.

Aura is nothing but a group of symptoms including nervous system - visual, mental, motor and speech. It lasts from several minutes to several hours 1-2 hours.

It is generally classified as prodrome, aura attack, post drome. But its not present in all migraine patients. Few might not get or notice all symptoms .

Prodrome

Symptoms before the main attack just 1-2 days prior, prodrome includes mental stress , nausea , visual blurriness, frequent eye movements, neck stiffness and constipation, more urination etc.

It is a very painful condition , it can last up to 72 hours .

It includes -

  1. Throbbing pain
  2. Usually on one side of head
  3. Sensitivity to light and sound
  4. Pulsation can be felt
  5. Triggered by stress or thoughts
  6. Visual impairment and nausea
  7. Tinnitus

Classification Of Migraine

Types

  1. Migraine without aura (common) (hemicrania simplex)
  2. Migraine without headache (silent only aura is present)
  3. Migraine with aura (complicated)
  4. Chronic migraine
  5. Retinal migraine (ocular migraine)
  6. Hemiplegic migraine (temporary paralysis and numbness)
  7. Status migrainosus
  8. Migraine with brainstem aura

Cause

Exact cause of migraine is not yet known , it is not due to a single cause or factor. It include multiple factors such as-

  • Genetics
  • Inflammation of vessels
  • Inflammation of Trigeminal nerves
  • Environmental factors

Pain Pathway

Pathway includes changes in the brainstem followed by irritation and inflammation of the trigeminal nerve. Which leads to chemical and hormonal imbalance in the brain. Disturbance in levels of serotonin. As serotonin is responsible for regulation of pain. Also imbalance in
𝘕𝘌𝘜𝘙𝘖𝘛𝘙𝘈𝘕𝘚𝘔𝘐𝘛𝘛𝘌𝘙𝘚.

Factors Which Triggers Migraine

  1. Stress ( Emotional factor)
  2. Hormonal changes in woman
  3. Missing a meal
  4. Physical strain and sleep changes
  5. Light and noise
  6. Medications
  7. Sensory stimuli

Symptoms-

It is similar to suryavarta .

Pain is felt in one half of the head. That is why it is called ardhhavbhedaka.
