Abstract
In 1933, Dawson described an entity he termed "inclusion body encephalitis," which was characterized by subacute encephalitis, loss of nerve cells, and Cowdry A inclusion bodies in neurons and glial cells. Van Bogaert (1945) reported a clinically similar disease in which the white matter of the brain showed intense fibrillary gliosis associated with a variable subacute inflammatory infiltrate, which he called "subacute sclerosing leukoencephalitis". Later, inclusion bodies were found in Van Bogaert's cases. It is now clear that both authors were describing the same disease, and further, that the disease is related to infection by the measles virus. The disease is now designated as subacute sclerosing panencephalitis. So, today we will read about SSPE in this article in brief.
Introduction
It is a condition of progressive neurological related disorder which affects mainly the central nervous system of the brain that leads to inflammation of the brain. The disease generally attacks children, although it can sometimes be seen in adolescents and young adults. The onset is heralded by personality changes, followed by involuntary movements.
Causes and Pathogenesis
It is caused by measles infection and disease diagnosed after many years of measles infection. The precise pathogenesis of SSPE is unclear, but several pieces of evidence suggest that lysis of neurons or oligodendroglia is induced not by direct action of the virus, but rather by antibody or cell mediated reactions directed against viral infected cells.
Symptoms
- Poor school performance
- Hallucination
- Insomnia
- Inflammation in brain
- Distract easily
- Motor function disturbed
- Temper outburst
- Suddenly muscular jerks of head, arms or other body part may occur
- Behavioral changes
- Intellectual problem
- Forgetfulness
- Ataxia
- Later, muscle rigidity increase and difficulty in swallowing
Diagnosis
On gross examination
The brain may be normal or firm and may contain regions of granularity and focal destruction.
On microscopic examination
There is a perivascular mononuclear cell infiltrate as well as Cowdry A inclusion bodies in neurons or oligodendroglia. Such inclusions have been shown to contain viral particles by electron microscopy. Neuronophagia and extensive neuronal loss are present in severely involved regions. Dense fibrillary gliosis follows as the process becomes older.
EEG
The EEG exhibits characteristic bursts of high-voltage activity. In most cases, the disease progresses relentlessly and devastates the brain, often leading to death.
CSF (cerebrospinal fluid)
An elevation of IgG is present in CSF, with an oligoclonal pattern and high levels of measles antibodies are present in the CSF of such patients.
Fluorescent antibody test
Measles antigen has been found in the neurons and glial cells of patients with SSPE by the direct fluorescent antibody test
Brain biopsy
Measles virus has also been cultured from the brain biopsy tissue of several cases of SSPE. It appears that the measles virus is present in the brain as an incomplete virus with defective replication.
Treatment
Subacute sclerosing panencephalitis has no treatment or cure available at present. If it is diagnosed in stage 1 then it may be possible to manage the condition by intraventricular interferon alfa and oral isoprinosine, but the effect of these medications vary from patient to patient. Only supportive treatment is given in this disease.
Ayurvedic overview
In Ayurveda, we can correlate SSPE with two main factors, one of the factors is vitiation of tridosha that is vata, pitta and kapha along with pranvaha srotas dusti and another is aagantuj vyadhi because it is caused by measles virus. Vitiation of vata takes place from either wasting of dhatus or obstruction in passage. Vata, pitta and kapha move in all the channels of the body. Due to subtleness, vata itself provokes the other two doshas that are pitta and kapha. When aggravated it excites them and carrying them to different places produces different disorders due to obstruction in passage and dries up rasa etc. This produces symptoms of all three doshas in this disease.
