This happened in August 2010, but I only just found out about it.
As described by the UK paper Metro, the scene was a shopping center in Daejon, South Korea.
Mr. Lee, a man in a motorized scooter let his anger get the better of him when he missed an elevator by a split-second because a woman inside had pressed the “close” button. Lee slammed the scooter into the elevator doors, but the elevator had left. Enraged, he then reversed his vehicle slightly and drove into the doors again, making a dent. Then he reversed back for one more attempt.
This time the doors gave way and he plunged 19 ft. to his death.

Police said the accident was caused by Lee’s anger at the woman not holding the door for him.
Shopping center officials vowed to strengthen the elevator doors to ensure the accident is never repeated.
