Amyloidosis is when an abnormal protein called amyloid which builds up in your organs and tissues. When it does, it affects their shape and how they work. Amyloidosis is a serious health issue that can lead to life-threatening organ failure. Amyloid isn’t normally found in the body, but it can be formed from many different types of protein. Organs that may be affected include the kidneys ,heart, liver, spleen, nervous system and digestive tract.
Amyloid fibrils are protein polymers which comprise identical monomer units (homopolymers). Functional amyloids play a beneficial role in a variety of physiologic processes (eg, long-term memory formation, gradual release of stored peptide hormones). Amyloidosis results from the accumulation of pathogenic amyloids—most of which are aggregates of misfolded proteins—in a variety of tissues
What are the symptoms?
In its early stages, amyloidosis may not cause any symptoms. When it becomes more severe, symptoms depend on the organ or organs affected.
If it affects your heart, you may experience:
- shortness of breath
- fast, slow or irregular heart rate
- pain in chest
- low blood pressure, which could cause lightheadedness
If it affects your kidneys, you may notice swelling in your legs due to fluid buildup (edema) or foamy urine from excess protein.
If your liver is affected, you may experience swelling and pain in the upper part of your abdomen.
If it affects your gastrointestinal tract, you may experience:
- nausea
- diarrhea
- Constipation
- appetite loss
- weight loss
- feeling of fullness right after eating
If it affects your nerves, you may experience:
- pain, numbness, and tingling in your hands, feet, and lower legs
- dizziness when standing up
- nausea
- diarrhea
- inability to feel cold or heat
General symptoms that can occur include:
- fatigue
- Skin changes. You may notice a waxy thickening of your skin; easy bruising of your face, eyelids or chest; or purplish patches around your eyes
- weakness
- bruising around your eyes or on your skin
- swollen tongue
- joint pain
- carpal tunnel syndrome, or numbness and tingling in your hands and thumb
What are the types and causes?
Bone marrow normally produces the blood cells in body which is to transport oxygen to your tissues, fight infections, and help your blood clot.
In one type of amyloidosis, infection-fighting white blood cells in the bone marrow produce an abnormal protein called amyloid. This protein folds and clumps, and is harder for the body to break down.
In general, amyloidosis is occurring by a buildup of amyloid in your organs. How the amyloid gets there depends on which type of the condition you have:
Light chain (AL) amyloidosis: this is the commonest type. It happens when abnormal amyloid proteins called light chains build up in organs like your kidneys, heart, liver, and skin. This type used to be called primary amyloidosis.
Autoimmune (AA) amyloidosis: This type mainly occur after infection like tuberculosis, or a disease that causes inflammation such as rheumatoid arthritis or inflammatory bowel disease. Most of the people with AA amyloidosis have rheumatoid arthritis. AA amyloidosis mainly affects your kidneys. Sometimes it also can damage your intestines, liver, or heart. This type used to be called secondary amyloidosis.
Dialysis-related amyloidosis: This type affects people who are on dialysis for a long period of time as a result of kidney problems. The amyloid deposits in the joints and tendons, causing stiffness and pain.
Hereditary (familial) amyloidosis: Hereditary amyloidosis is rare. It can be passed in generation within a family. The proteins produced in hereditary amyloidosis may cause problems with the heart and may cause carpal tunnel syndrome and eye abnormalities.
Senile amyloidosis: Affects the heart in older men.
Risk factors
Factors that increase your risk of amyloidosis include:
Age. Most people of age 60-70 are diagnosed with amyloidosis, although earlier onset occurs.
Sex. most commonly in men.
Other diseases. Having inflammatory disease or chronic infectious increases your risk.
Family history. Some types of amyloidosis are hereditary.
Kidney dialysis. Dialysis can’t always remove large proteins from the blood. If you’re on dialysis, abnormal proteins can build up in your blood and eventually be deposited in tissue. This condition is less common with more modern dialysis techniques.
Race. People of African descent appear to be at higher risk of carrying a genetic mutation associated with a type of amyloidosis that can harm the heart.
