The couple's first breakthrough in the world of business was the founding of Elzac, together with their financial partner Zachary. The company was active in the 1940s, and it produced figural costume jewelry brooches.

Handler aspired to invent a doll that would better facilitate the way young girls were playing with their dolls. Handler and Mattel introduced Barbie, the teenage fashion model, to skeptical toy buyers at the annual Toy Fair in New York on March 9, 1959.

Ruth Mosko Handler is best known as the inventor of the Barbie doll, but her most important work may be her prosthetics for survivors of breast cancer.

She was named the Women of the Year in Business by Los Angeles Times and was included in the Toy Industry Hall of Fame by Toy Manufacturers of America. She also received the 'Volunteer Achievement Award' by the America Cancer Society. She was the first ever 'Woman of Distinction' of the United Jewish Appeal.

In 1978, she and several other former officers of Mattel were indicted on charges of fraud and false reporting to the Securities and Exchange Commission. She pleaded no contest to the charges, which investigators said resulted from attempts to influence stock prices, and was fined and sentenced to community service.
