It all goes to show that the writers are not backing away from the tradition of basing the stories in the psychology of the Brothers Winchester and their long list of massive personal issues. This episode said a lot about Sam and Dean are where they stand with one another at the moment. They are on better terms, but both of them have reasons not to trust completely.
