After graduating from his school, A&M Consolidated High School, he joined A&M University where he played for the Texas A&M Aggies. He had a very good college career. During his college days, Alex Caruso played 137 games for Texas A&M Aggies, where he averaged 8.0 points, 4.7 assists, and 2.02 steals per game. Alex Caruso ended his college career becoming the all-time leader in assists with 649 assists.

Alex Caruso turned professional on 23 September 2016 when he signed for the Oklahoma City Thunder. He went undrafted in NBA 2016 draft but later joined the Philadelphia 76ers for the NBA Summer League. On 3 November 2016, he was acquired by the Oklahoma City Blue of the NBA development league.
