Reserve team boss Luis Castro praised him for having "extraordinary mental qualities, to go along with technical and tactical skills" during his time in the development structures.

Neves was planned to skip Porto's under-19s and play with the reserves instead before the 2014-15 season. But after teammate Mikel Agu was hurt, Julen Lopetegui asked him to participate in preseason training with the main squad.

He made his Primeira Liga debut on August 15, 2014, when he was 17 years and 5 months old. He started and scored in a 2-0 home victory over Martimo, becoming the club's youngest player ever to do so.

Neves sustained a right knee injury on December 10, 2014, during a Champions League match against Shakhtar Donetsk as a result of a contact with Alex Teixeira in the middle of the field.

His injury was described as a sprain with apparent damage to the internal lateral ligament by the team after the game.

In the group stage match between Porto and Maccabi Tel Aviv on October 20, 2015, Neves, who was then 18 years and 221 days old, broke Rafael van der Vaart's previous record by starting as captain. Rafael van der Vaart had held the record since September 16, 2003.

On July 8, 2017, Neves agreed to a deal with Wolverhampton Wanderers of the EFL Championship for an unknown sum, which is estimated to be in the neighborhood of £15.8 million, setting a club and league record.

Neves' new deal, which runs through June 2023, was announced by Wolves in July 2018. In the opening game of the new campaign, he not only helped Ral Jiménez in his professional debut but also scored his first Premier League goal as his club twice overcame deficits to draw 2-2 with Everton.

Neves earned his 50th appearance for the Wolves on October 6, 2018, and he celebrated by helping set up Matt Doherty's game-winning goal in a 1-0 victory over Crystal Palace.

Neves was appointed Wolves' club captain for the first time on September 25, 2019, when he led the team in Conor Coady's absence as Wolves advanced to the 2019-20 EFL Cup fourth round with a penalty shootout victory over Reading at Molineux.

On February 13, 2022, Neves made his 200th competitive appearance for Wolves in all competitions during a 2-0 Premier League victory away at Tottenham Hotspur.

On March 13, the day of his 25th birthday, he helped Coady score the lone goal in Wolves' victory against Everton, which marked the club's first league double over that opponent since 1972-73 and marked Wolves' 1,000th win in top-flight football (over 67 seasons dating back to 1888).

Neves assumed the captain's role against Leeds United on August 6, 2022, keeping Conor Coady on the bench for substitutes. This was the team's opening game of the 2022-23 season.

After the defender left on loan to Everton, he was named the full-time captain of Wolves.
