Impetigo

  • It is an old Skin disease. Its name dates back to 14th-century England and comes from the Latin word impetere i.e. "to attack" or the easily spread infection.
  • In Ayurveda, it comes under the TWAKA VIKARA'S or the skin disorders due to Agantuja or external factors affecting your body.
  • Favorable condition to this Bacteria is hot, moist conditions. It tends to be seasonal, peaking in the summer and fall in northern climates. In the areas with warm and humid climates, it tends to occur year-round.
  • Prevalence - According to some study by a trusted source it is estimated 162 million children worldwide have this disease at any one time. In India, it is Very common i.e. more than 10 million cases per year.
  • It is more common in developing countries and in poor areas of industrial countries.

Understanding Impetigo

DEFINITION- It is a common and contagious skin infection caused by Bacteria that infect outer layers of skin or the epidermis presenting as a weepy skin rash or lesion with a red base that may develop into a yellow or honey-colored crust or scab.

    • Incidence- Effect both the races equally but most commonly affects to the infant and children (2 to 5yrs of Age).
    • Beginning Stage - It often starts in minor cuts, insect bites , or a rash such as eczema and broken skin mostly but can also occur on your healthy skin.
    • Primary impetigo- If it infects the healthy skin
Note- It isn't always easy or necessary to make this distinction.
    Triggering factor- Poor hygiene or warm temperature

TYPES OF IMPETIGO

  • Nonbullous- This is the most common form in adults. In this there is formation of Crusts.
  • Bullous - This causes large blisters on the skin.
  • Ecthyma -Itis serious type of impetigo form that usually results from untreated impetigo and causes ulcerative sores that goes down into the deeper layers of the skin.

NIDANA OR THE CAUSES -

  • Causative Organism-Staphylococcus aureus or S.Pyogenes
  • Most affected sites- The face, arms, and the legs
  • Impetigo is an infection caused by strains of the bacteria.
  • These bacteria can get into your body through a break in the skin from a cut, scratch, insect bite, or rash. Then they can invade and colonize.

CONTAGIUS OR NON-CONTAGIUS??

  • It is Highly Contagious disease i.e. If you come in contact of the sores of a person with impetigo or you come in contact with the items like towels, clothes or sheets that the person had used.
  • In the environment, these bacteria may also be present. and it is not necessary to develop among all the people.
  • In some people Staphylococcus may be present inside of their nose and may get infected if the bacteria spreads to their skin.
Adults and children are at higher risk for impetigo and underlying the following factors-
  • live in a warm and humid climate
  • Suffering from Diabetes Mellitus
  • If they are undergoing dialysis
  • HIV or Compromised immune system
  • Skin conditions such as eczema, dermatitis, or psoriasis
  • Sunburn or other burns
  • Itchy infections such as lice, scabies, herpes simplex or chickenpox
  • Insect bites or poison ivy
  • Playing a contact sport

SIGNS AND SYMPTOMS-

  • First signs of impetigo is the Reddish sores on the skin, Clustered around the nose and lips mostly.
  • The sores quickly grows into blisters then oozes and burst and then form a yellowish crust.
  • The clusters of blisters may expand to cover more of the skin. Also the red spots just develop a yellowish crust and absence of blisters.
  • The infection is usually not so serious, but complications may develop in some cases.
  • The sores - Itchy and occasionally painful. After the crust phase, they form red marks that fades without leaving scars.
  • In Infants sometimes they have a less common type of impetigo, with larger blisters around the diaper area or in skin folds. The fluid-filled blisters soon burst, leaving a scaly rim known as a collarette.
  • Sometimes, it may involve swollen glands in the area of the outbreak or a fever thus making you uncomfortable.

AYURVEDIC ASPECTS OF IMPETIGO

In Ayurveda, Impetigo can be enlisted under Twaka Vikara and compared with a type of Raktaja Sukshma Krimi related disease.

  • Oupasargika roga- Non- bullous(Crusted) type of impetigo comes under this category.
  • In this disease there is involvement of both Kapha and Pitta i.e. Kapha-Pitta Samsargaja in the majority of the lesions. Pitta dosha the main features are Raga and Paka.

Chikitsa or Treatment- The basic line of treatment is Kapha-Pittahara chikitsa

In the aashukari state of the diseasethere is involvement of pitta-rakta then krimighna chikitsa should be administered, as the disease belongs to oupasargika roga., along with bahya parimajana chikitsa is plays an important role.
