Treatment of Prurigo Nodularis with Ayurveda – Symptoms & Diagnosis

Introduction

Prurigo Nodularis is also known as Picker Nodules and Hyde's disease. PN is an uncommon auto immune skin condition wherein hard crusty lumps are formed on the skin which causes severe itching. Prurigo means itching and nodularis means nodules. The itching is so severe that it stops only once the PN is scratched and it starts bleeding. It is a form of chronic inflammatory dermatosis. Itching leads to distraction. The scratching leads to formation of hyperpigmented lesions with scaly surface and is known as lichen simplex chronicus.

PN is usually in the areas easily reached like arms, shoulders and legs. The factors that trigger PN are related to nervous and mental conditions of the patient. It could be due to improper liver and kidney functions, being anemic, having HIV infection and even skin problems like eczema. Family history of atopic dermatitis or asthma are mostly victims of PN. Nodular Prurigo are known to occur in symmetry and are usually grouped together. Poor and incorrect diet that includes excessive intake of salt, sour or pungent foods, excess sugary or oily foods or overuse of cosmetics, perfumes etc. are other factors leading to PN.

The problem is common in middle aged and older women compared to men.According to Ayurveda, skin problems are more common with people having Pitta imbalance as Pitta can overheat the blood and predispose to toxin conditions.

SYMPTOMS OF PRURIGO NODULARIS

  • Discrete, scaly, firm, open nodules are present on extensor surfaces of legs and arms.
  • The sore is hard and about half an inch. It is dry and rough to touch.
  • Due to repeated scratching and rubbing, the skin gets dark (hyperpigmentation) and thick (lichenification).
  • The problem often leads to stress and depression in patients.

DIAGNOSIS OF PRURIGO NODULARIS

PN can easily be diagnosed by visual examination as there are nodules with intense itching episodes. Skin biopsy is done to confirm PN. Blood tests are performed to check the blood count along with liver, kidney and thyroid function tests. Patch testing is done in some cases. Other tests include Dermoscopy to differentiate various nodular pruritic conditions and direct immunofluorescence can be performed.

AYURVEDIC TREATMENT OF PRURIGO NODULARIS

Prorigo nodularis is a very challenging problem to treat. Antihistamines, cortisones, antibiotics are prescribed in most cases.
